IN THE MATTER OF OHIO COUNTY CASE NO. 14-CJA-88; MISTY SISARCICK, biological mother and primary custodian of infants C. S., whose date of birth is September 13, 2010 and RYAN SMITH, alleged biological father of infant C. S., whose date of birth is September 13, 2010. Natural child of MISTY SISARCICK.
The above action may result in a termination of parental rights of RYAN SMITH, alleged biological father of infant C. S., whose date of birth is September 13, 2010. The Petitioner, the West Virginia Department of Health and Human Resources has filed a Petition that may result in the termination of parental rights of RYAN SMITH, alleged biological father of infant C. S., whose date of birth is September 13, 2010.
The Petitioner alleges that RYAN SMITH, alleged biological father of infant C. S., whose date of birth is September 13, 2010, has abandoned and /or failed and neglected to provide for the care, custody and control of infant C. S., whose date of birth is September 13, 2010, and has made no effort to provide for said care of infant, or maintain the infant in any way from that time until present. It is ORDERED that RYAN SMITH, alleged biological father of infant C. S., whose date of birth is September 13, 2010 is notified that he may obtain a copy of the Petition from the Clerk of the Circuit Court of Ohio County, West Virginia and he is further ORDERED to appear in the office of the Honorable James P. Mazzone, Judge of the Circuit Court of Ohio County, West Virginia, Fifth Floor, Ohio County Courthouse, Wheeling, West Virginia on the 16th day of June, 2015 at 10:30 a.m. for an Adjudication Hearing and object or make other defense to the petition, otherwise, parental rights of RYAN SMITH, alleged biological father of infant C. S., whose date of birth is September 13, 2010 may be terminated.
