It is ORDERED that the Respondents shall appear at the Department of Health and Human Resources, 1236 North State Route Two, New Martinsville, Wetzel
County, West Virginia, to be transported to the dental appointment of S. B. (DOB 09/11/2014) on the 15th day of July, 2020 at 6:00 o’clock A.M. The Honorable David W. Hummel, Jr. has determined this vital procedure for the health and welfare of S. B. requires your presence as her parents.
Failure to appear as ordered by the Court is punishable as Contempt of Court under West Virginia Code ยงยง57-5-6 and 61-5-26. Specifically, your failure to
appear, as ordered, shall result in a capias issued for your arrest to address the Contempt of Court.
Further, each of you should call your court-appointed counsel at his office.
A copy of the Notice and Affidavit by counsel for the Department is available at the Wetzel County Circuit Clerk’s Office.
