RICHARD MORGAN PATIENT RECORDS LEGAL NOTICE If you were a patient of Richard Morgan, and visited him for substance use disorder treatment at 4834 MacCorkle Ave. SW in South Charleston, West Virgina, this publication serves as notice that your patient records regarding substance use disorder treatment, may have been disclosed to the West Virginia Attorney General Medicaid Fraud Control Unit as part of an ongoing investigation. Pursuant to Section 42 of the Code of Federal Regulations, Part II, the Medicaid Fraud Control Unit obtained a court order allowing the disclosure of your patient records to investigate and/or prosecute a substance use disorder treatment provider, and/or the holder of the patient records. As such, the disclosure of these records is not for the purpose of investigating or prosecuting any patient, and the records cannot be used to pursue action against any patient whose records were disclosed. As the patient, you have a right to object to the disclosure of your patient records. This objection is limited to whether the Court Order authorizing disclosure was properly obtained.
