Southwest Virginia Health Authority 851 French Moore Jr. Boulevard, Suite 178 Abingdon, Virginia 24210 NOTICE OF RECEIPT OF APPLICATION FOR A COOPERATIVE AGREEMENT BY WELLMONT HEALTH SYSTEM AND MOUNTAIN STATES HEALTH ALLIANCE Take notice that pursuant to Section 15.2-5384.1(C)(2) of the Code of Virginia, as amended from time-to-time (the “Code”), the Southwest Virginia Health Authority received a completed application for the “Commonwealth of Virginia Application for a Letter Authorizing Cooperative Agreement” from Wellmont Health System and Mountain States Health Alliance (the “Application”) on August 26, 2016. All interested persons who desire to comment on the Application shall file such comments on or before September 30, 2016 with the Authority by writing to the Authority at 851 French Moore Jr. Boulevard, Suite 178, Abingdon, Virginia 24210. As required by the Code, any person delivering written public comment must provide their name and mailing address. The public may submit written comments regarding the Application for a period of twenty (20) days, commencing on September 1, 2016 and ending September 30, 2016.
