NOTICE As required by 40 CFR 403.8 the Huntington Sanitary Board hereby provides the names of the Industrial Users (IUs) which were in significant noncompliance at any time during the 12-month period from July 1, 2021, to June 30, 2022: Industry: Huntington Alloys, Corporation Address: 3200 Riverside Drive, Huntington, WV 25705 Violation: Meeting the Technical Review Criteria for Significant Non-Compliance for the monthly mass limit of Chromium for the second and third quarters at monitoring point 001. Action Taken: Huntington Alloy, Corporation was issued a Notice of Violation for the mass daily limit of Chromium and an Administrative Order for the mass daily and mass monthly limit of Chromium requiring resampling and a plan of corrective action. Violation: Meeting the Technical Review Criteria for Significant Non-Compliance for the mass daily limit for Zinc for the fourth quarter at monitoring point 004. Action Taken: Huntington Alloy, Corporation was issued a Notice of Violation requiring resampling and a plan of corrective action. Industry: Steel of West Virginia, Inc. Address: 18th Street and Commerce Ave., Huntington, WV 25701 Violation: Meeting the criteria for Chronic Significant Non-Compliance for the concentration limit for Oil and Grease for quarter 2 and quarter 3 at monitoring point 006. Action Taken: Steel of West Virginia, Inc. was unable to resample for this parameter because it occurred on an annual pond cleanout. They were required to submit an explanation and a corrective plan of action for future cleanouts. Violation: Meeting the Technical Review Criteria for Significant Non-Compliance for the mass daily limit of Zinc at monitoring point 002 for the first and third quarter, and Chronic Significant Non-Compliance for the mass daily limit of Zinc for the second quarter at monitoring point 002. Action Taken: Steel of West Virginia was issued two Notices of Violation requiring resampling and a plan of corrective action. Industry: Huntington Plating Address: 625 Monroe Ave., Huntington, WV 25704 Violation: Meeting the criteria for Chronic Significant Non-Compliance for the concentration limit for Phenolics and the mass daily limit for Zinc in quarter 2 and meeting Technical Review Criteria Significant Non-Compliance for the parameter of Zinc mass daily limit and the concentration limit for Phenolics for quarter 3 at monitoring point 001. Action Taken: Huntington Plating was issued a Notice of Violation for the parameter of Zinc and the parameter of Phenolics which required them to resample for each parameter and submit a corrective plan of action. Industry: Valicor Environmental Services, LLC Address: 515 8th Ave. West Bay 100, Huntington, WV 25701 Violation: Meeting the Technical Review Criteria for Significant Non-Compliance for the daily concentration limit for bis (2-Ethylhexyl) phthalate for quarter 2 at monitoring point 001 and meeting the Technical Review Criteria for Significant Non-Compliance for the monthly concentration limit for bis (2-Ethylhexyl) phthalate for quarters 3 and 4 at monitoring point 001. Action Taken: Valicor Environmental Services, LLC was issued a Notice of Violation for the daily and monthly exceedance and was required to resample and submit a corrective plan of action. Violation: Meeting the Technical Review Criteria for Significant Non-Compliance for the daily concentration limit for Total Toxic Organic for quarter 4 at monitoring point 001. Action Taken: Valicor Environmental Services, LLC was issued a Notice of Violation for the daily concentration limit of Total Toxic Organic and was required to resample and submit a corrective plan of action. Industry: Veterans Administration Medical Center Address: 1540 Spring Valley Dr. Huntington, WV 25704 Violation: Meeting the criteria for Chronic Significant Non-Compliance for the instantaneous measurement of pH for quarter 3 at monitoring point 001.
