PETITION FOR CHANGE
OF NAME OF ADULT
Now comes the Petitioner and pursuant to West Virginia Code § 48-25-101 as amended, petitions this court to change the Petitioner’s name from CODY JON DURNELL to CODY JON KERNS. In support thereof, the Petitioner states as follows:
1. The Petitioner is currently over the age of eighteen (18) years, having been born on May 14, 2000.
2. The Petitioner has been a bona fide resident of Wood County, WEst Virginia for at least one (1) year prior to the filing of hit Petition.
3. The name change is not for the purposes of avoiding debt or creditors.
4. The Petitioner is not a registered sex offrender pursuant to any state or federal law.
5. This name change is not for purposes of avoiding any state or federal law regarding itdentiy.
6. This name change is not sought for any improper or illegal purposes.
7. The Petitioner is not a convicted felon in any jurisdiction.
8. This name change is not sourght for any purpose of evading detection, itentification or arrest by any local, state or federal law-enforcement agency.
The Petitioner was name CODY JON DURNELL at birth, and has been, and is now, known as CODY JON DURNELL.
10. The Petitioner seeks this name change so that his surname matches that of his biological father.
WHEREFORE, the Petitioner prays that the Court declare that he shall henceforth be known as CODY JON KERNS; and for such other and further relief as this Court may deem just and proper.
