TO Stephen Thomas Cox the purported father of M.M. DOB April 24, 2018 be advised that a matter is pending in the Circuit Court of Taylor County, West Virginia, as case number 18-JA-104 involving your parental rights, if any, to a female child whose date of birth is April 24, 2018. You must appear at a hearing scheduled for December 6, 2018 at 9:00 am at the Taylor County Circuit Court, at 214 West Main Street, Grafton, WV and/or defend any such rights within 15 days by serving a response upon the Taylor County Circuit Clerk or the Taylor County Assistant Prosecuting Attorney, Shawn Nines, whose address is 222 West Main Street, Grafton, West Virginia, 26354. If you fail to do so, judgment by default will be taken against you, your parental rights may be forever terminated, and you may thereafter be barred from asserting any future claims for parental rights.
