You have been named as the parents of one or more of the children who are subject of the above styled matter involving a petition for Family Infant Guardianship, of the child whose mother is KAYLA NICOLE DENT and whose father is CHAD WARREN HOLLEY.
It appearing after due diligence the Petitioner, KELLY LYNN GREEN whose address is 7685 SPRUCE LAUREL ROAD, CLOTHIER, WV 25047, has been unable to locate your current whereabouts. These proceedings can result in the permanent guardianship of your children with KELLY LYNN GREEN if you fail to file a response or make an appearance in this matter A Permanent Guardianship Hearing is scheduled in this matter for th 14TH day of JANUARY, 2018 at 8:45 a.m. in the Boone County Family Court, at which time and place you may appear and make any objection known or otherwise protect your interest. Your consent to this proceeding is NOT necessary; however, please be advised that your custodial rights to your child(ren) may be adversely affected by this hearing.
