Z.H. 20-JA-122-1 Z.H. 20-JA-123-1 Z.M. 20-JA-124-1 Z.M. 20-JA-125-1 K.H. 20-JA-126-1 K.H. 20-JA-127-1 TO: ALICIA HENRY / TAYLOR WASHINGTON / YAHFATT FROST and UNKNOWN FATHER The purpose of the above action is to determine guardianship of the child, Z.H., born March 20, 2008; the child, Z.H., born April 26, 2011; the child, Z.M., born February 22, 2019; the child, Z.M., born February 22, 2019; the child, K.H., born June 7, 2020, and the child, K.H., born June 7, 2020, to Alicia Henry. The petitioner, the West Virginia Department of Health and Human Resources, is seeking permanent legal and physical custody of the said infant children. It is ORDERED that Alicia Henry, the biological mother of the aforementioned children; Taylor Washington, the biological father of the aforementioned child, Z.H.; Yahfatt Frost, the biological father of the aforementioned child, Z.H.; and the Unknown Father of the aforementioned children, K.H. and K.H., appear in the Harrison County Circuit Court, Division I, 4 th Floor, Harrison County Courthouse, Clarksburg, West Virginia, on the 28 th day of September, 2020 at 2:00 o’clock p.m., or as soon thereafter as it may be heard, for the adjudication of Alicia Henry, Taylor Washington, Yahfatt Frost and the Unknown Father of the infant children, K.H. and K.H. Said persons are instructed that they have thirty (30) days from this Notice to answer the Petition herein. Said persons are given notice that they must object, or, otherwise permanent legal and physical custody of the said infant children may be granted to the West Virginia Department of Health and Human Resources and parental rights may be permanently terminated.
