BOX 100, OAK HILL, WV 25901 has submitted an application to the Department of Environmental Protection (DEP), 254 Industrial Drive, Oak Hill, WV 25901, for a significant permit revision to Article 3 permit number S301601 in order to construct a combined coal refuse disposal embankment on the deck of an existing rock valley fill. The revised permit will discharge into Unnamed Tributary of/and Open Fork, Unnamed Tributary, Unnamed Tributary of Taylor Branch, all of Loop Creek (Receiving Stream) of KANAWHA RIVER (Major Subbasin) and is located 2.3 (miles), East (Direction) of Kincaid (nearest PO), in the Valley District of Fayette County, Longitude 81 f 13′ 27.0000″ and Latitude 38 f 2′ 8.0000″ (Coordinates from USGS Topographic Map).
