To operate and maintain a treatment and disposal system and best management practices for the direct discharge of treated stormwater associated with construction activities via multiple outlets to unnamed tributaries (UT) of Haddix Run, UT of UT of Fools Run of Haddix Run, UT of Fools Run of Haddix Run, Fools Run of Haddix Run, UTs of/and UTs of/and Goodwin Run of Haddix Run, UTs of/and UTs of/and Sugarcamp Run of Shavers Fork, an UT of/and UTs of/and Smokey Hollow of Shavers Fork, Shavers Fork, Several UTs and their associated headwaters of Cheat River, and Cheat River.
Business conducted:
314.2 acres of earth disturbance for the construction of approximately 4.13 miles of 4-lane highway and all
associated access roads, storage and maintenance areas associated with the project.

On the basis of review of the application the “Water Pollution Control Act (Chapter 22, Article l l-8(a)),” and the “West Virginia Legislative Rules,” the State of West Virginia will act on the above application.
Any interested person may submit written comments on the site registration permit application and may request a public hearing by addressing such to the Director of the Division of Water and Waste Management within 30 days of the date of the public notice. Such comments or requests should be addressed to:

The public comment period begins October 21, 2020 and ends November 20, 2020.
Comments received within this period will be considered prior to acting on the permit application. Correspondence should include the name, address and the telephone number of the writer and a concise statement of the nature of the issues raised. The Director shall hold a public hearing whenever a finding is made, on the basis of requests, that there is a significant degree of public interest on issues relevant to the site registration permit application and this facility’s coverage under the General Permit. Interested persons may contact the Public Information Office to obtain further information.
The application may be inspected, by appointment, at the Division of Water and Waste Management Public Information Office, at 601 57th Street SE, Charleston, WV, between 8:00 a.m. and 4:00 p.m. on business days. Copies of the application and the General Permit and Fact Sheet may be obtained from the Division at a nominal cost.
