We are required to monitor your drinking water on a regular basis. Results of regular monitoring are an indicator of whether or not our drinking water meets health standards. Between 10/1/2021 and 12/31/2021, we failed to submit the required samples as indicated below and therefore cannot be sure of the quality of our drinking water during that time.
What should I do?

Fourth Quarter monitoring was pulled late as I misread the sampling schedule. I was under the impression that the sample could be taken at any point in the quarter but it needed to be pulled before November 30.
I have created a sample board to post all required samples and sampling times, this mistake will not be made again.
