You are hereby notified that your parental rights to Madisyn Alaina Cook, if any, may be terminated in this adoption proceeding, unless you appear and defend any such rights within 30 days of first publication of this Notice, by filing a Response with the Circuit Clerk of Raleigh County, West Virginia and by serving said Response upon counsel for Petitioners, Patricia J. DiFranco and Legal Aid of West Virginia, 115B South Kanawha Street, Beckley, WV 25801.
You are further notified that a final hearing will be held on the 3rd day of June, 2019 at 1:30 p.m., in the Raleigh County Circuit Court, 222 Main Street, Beckley, West Virginia, before the Honorable Darl W. Poling.
Failure to respond or appear will result in termination of any parental rights you may have.
