NOTICE
Please take notice that a Class Action Settlement has been reached in Kolovich et al. v. Logan General Hospital, LLC, and Raleigh General Hospital, LLC pending in the Circuit Court of Logan County, West Virginia (Civ. Action No. 20-C-31 & 51). If you were a patient sometime between April 16, 2016, and May 1, 2020 you may be entitled to a one time monetary payment between $11 and $15 from the settlement fund. Defendants deny any wrongdoing. The amount you receive will depend on how many class members remain in the settlement or choose to exclude themselves. Logan General Hospital, LLC or Raleigh General Hospital, LLC will create a settlement fund of $3,700,000.00 to pay these claims. Class Counsel are requesting compensation from the settlement fund and reimbursement of costs for bringing the lawsuit. Class Representatives are requesting a compensation award from the settlement fund.
You have the choice to opt out of this lawsuit (allowing you to pursue any claims on your own). If you do not opt out, you have the right to object to the terms of the proposed settlement, but you will be bound by the settlement if it is finally approved by the Court and by any other orders of the Court in the case.
