HIGH TECHNOLOGY FOUNDATION LEGAL NOTICE The Association Health Insurance Plan of the Affiliate Services Group headquartered within the I-79 Technology Park, Fairmont, WV 26554 (the “Association Plan’) seeks proposals for insurance brokerage for an existing HighmarkWV Blue Cross / Blue Shield Association Health Insurance Plan, providing coverage to approximately 1,300 insured employees and 3,000 members (the “Insurance Brokerage Services”). Any insurance broker with experience in association plan brokerage, claims submission, member recruitment, legal compliance and related support services for employers’ health insurance plans are invited to submit their proposal, including capabilities, experience, proposed services and cost. Interested brokers may obtain request for proposal (“RFP”) specifications, by email, at [email protected] (in which case the RFP specifications will be emailed on October 21, 2015). Alternatively, interested brokers may obtain the RFP specifications by web access on or after October 21, 2015, at www.affiliateservices.org. Email requests for RFP specifications shall be submitted not later than 12:00 Noon, October 20, 2015.
