Sealed Bids for Contract Valley Health Care – One Story Addition will be received by the Valley Health Care Inc, 1 Poling Street, Mill Creek, Randolph County, West Virginia 26280, until 4:00 p.m., L.P.T., on Tuesday, February 10, 2015, for furnishing labor and materials and performing all Work set forth in the Contract Documents prepared by The Thrasher Group, Inc. Immediately following the scheduled closing time for the reception of Bids, all proposals which have been submitted in accordance with the conditions of the Contract Documents will be publicly opened and read aloud.

The approximate quantities of Work to be Bid upon is as described as follows:

In general the Contractor is to provide all labor, materials, equipment, fees, insurance, taxes and similar contractor expenses as required for the construction of a one story, wood framed, brick veneered 2,111 SQ FT. addition to the existing 5,315 SQ FT Valley Health Care Facility in Mill Creek, WV.

The Work will be substantially completed within 200 Calendar days after the date when the Contract Times commence to run, and completed and ready for final payment within 230 Calendar days after the date when the Contract Times commence to run. Liquidated damages shall be $500.00 per day.

• Bidding Documents issued as hard copy drawings and hard copy specifications are available for $125.00 per set.
• Bidding Documents issued as digital drawings and digital specifications on a C.D., are available for $50.00 per set.

Amounts paid are not subject to refund.

A two envelope system will be used.

Envelope No. 1 must have the following information presented on the front:

Received by: Valley Health Care, Inc.

Envelope No. 2 labeled “Bid Proposal” shall also be placed inside of Envelope #1.

Envelope No. 1 will be opened first and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR – 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 “Bid Proposal” , will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order, Envelope No. 2 “Bid Proposal” will not be opened and the Bid will be considered non-responsive and will be returned to the Bidder.

A Bidder may not withdraw his Bid for a period of sixty (60) days after the date set for the opening of Bids.

Bids shall be accompanied by a certified check or Bid Bond payable to the Valley Health Care, Inc., an amount equal to five percent (5{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}) of the base Bid.

Bidders must hold a current West Virginia contractor’ s license on the date of Bid Opening.

Valley Health Care, Inc.’ s (VHC) project is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) through Grant Number: C8CCS27624 and Program Title: Affordable Care Act Patient Center Medical Home – Facility Improvements Grant Program. The Grant providing funding for the Valley Health Care, Inc. project is for the amount of $250,000 which constitutes approximately 35{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c} +/- of the total Project Funding. VHC will provide approximately $466,500 (65 {4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}+/-) of the total project cost. VHC and grant project funding can and/ or will be used to support project expenses as follows; Administrative and legal; Land, structures and rights-of-way, appraisal, etc.; Relocation expenses and payments; Architectural and engineering fees; Other architectural and engineering fees; Project inspection fees; Site work; Demolition and removal; Construction; and Equipment purchases.

Bids received after the scheduled closing time for the reception of Bids will be returned unopened to the Bidders.

Valley Health Care, Inc. reserves the right to reject any and all Bids.

A pre-bid conference will be held at the Valley Health Care Administrative Building located at 1 Poling Street, Mill Creek, Randolph County, West Virginia, on Thursday, January 29, 2015, at 9:00 a.m., L.P.T.
