NOTICE Request for Proposal Refrigeration Preventive Maintenance / Service The Tazewell County School Board is accepting “Price Quotes” for the preventive maintenance service of the refrigeration equipment located in the school kitchens / cafeterias at fifteen (15) different locations. General liability insurance in the amount of one million dollars ($1,000,000) will be required. Please contact George Godbey at 276.988.5511 ext. 1503 or David Beavers at ext. 1501 for additional information. Quotes will be received no later than 2 pm on April 30, 2015. GENERAL: Please submit with your “Price Quote”: (1) Lump sum cost for the entire scope of service: (2) an hourly labor and mileage rate for services after normal working house, weekends, and holidays: (3) a reference list of at least five (5) current customers having similar equipment, including contact names and phone numbers: (4) an emergency procedure (phone number, contact name, etc.) for after hours, holidays and weekend repairs: and (5) a current copy of your companies general liability insurance coverage. An inventory list of equipment by location, will be provided, however it shall be the prospective bidder’s responsibility to verify all equipment prior to submitting their “Price Quote”. SCOPE: Thoroughly clean, adjust and lubricate system components (i.e. coils, motors, starters, drives, condensers, etc.); accomplish refrigerant leak tests and recharge as needed; check and replace as needed all starter contacts; tighten all terminals, calibrate thermostats and controls, check amperages; inspect all filters and replace as necessary. Ensure the equipment is working properly. When submitting invoices please include your service report that states the date of inspection, work performed, part replaced, etc. Also note any items that need repaired and estimated costs. The contact period will begin July 1, 2014 and end on July 30, 2016. The contract may be renewed by Tazewell County Public Schools for two successive one (1) year periods, under the terms and conditions of the current contract, and at a reasonable time prior to expiration. A vendor will be selected primarily based n their technical capability / qualifications, performance records, and experience. Price shall be considered, but not the sole determining factor. (Va. Code 2.2-4301) FREQUENCY: All locations shall be serviced three (3) times during the physical year; June, November, and April (while school is not in session). PROCEDURE: The Service Technician must contact the Head Custodian at each location and inform them that they are there to perform services to the refrigeration equipment. After completion of the service the Technician shall provide the Head Custodian with a copy of the service document stating: (1) date; (2) time; (3) equipment serviced; (4) location (school name); (5) description of service performed; (6) hours worked and any other pertinent information. One copy of the document is to be retained by the Cafeteria Manager and one copy sent to the Food Services Department located at 328 Advantage Drive, Tazewell, VA 24651 along with the invoice. NOTE: The successful vendor will be required to comply with School Board policy DJF (attached). Tazewell County Public Schools reserves the right to reject any or all quotes.
