The object of this suit is to obtain an Order from the Circuit Court of Wood County, West Virginia, to allow the adoption of BABY BOY KELLEY, born December 6, 2014, and to change the name of BABY BOY KELLEY which suit may result in the termination of the parental rights of the birth mother of said child.
TO ANYONE CLAIMING TO BE THE BIRTH MOTHER OF SAID CHILD:
You are hereby notified that a Petition for Adoption has been filed with the Clerk of the Circuit Court of Wood County, West Virginia, and that you may serve upon KATHY C. THOMAS, Petitionersá Attorney, whose address is 813 Grand Central Avenue, Suite B, Vienna, West Virginia, 26105, a responsive pleading to the Petition for Adoption filed in this action on or before November 6, 2015. If you fail to do so, thereafter judgment by default may be taken against you for the relief demanded in the Petition for Adoption and your parental rights, if any, may be terminated in this proceeding. You may appear and defend any such rights within the required time after such service.
You are further notified that a hearing on the Petition for Adoption is scheduled for November 9, 2015, at 3:00 oáclock, p.m., before the HONORABLE J. D. BEANE, Judge of the Fourth Judicial Circuit, in this courtroom at the Wood County Judicial Building, Fourth Floor, 2 Government Square, Parkersburg, West Virginia, but, if you do not respond as legally required by November 6, 2015, you may be prevented from taking part in or objecting to such proceedings.
