Application to amend P.S.C. M.C. CertiEcate No. 7572 to authorize to provide non-emergency medical transport in of Calhoun, Doddridge, Gilmer, Jackson, Pleasants, Ritchie, Roane, Tyler, Wetzel, Wirt and Wood Counties in West Virginia.
COMMISSION CORRECTIVE PUBLICATION ORDER
On June 16. 2016, G & B Transportation, LLC filed an application for the amendment of P.S.C. M.C. Certificate No. 7572. The proposed amendment is as follows:
to operate as a common carrier by motor vehicle in the transportation of passengers in limousine service, through the use of specialized limousines that provide specialized multipassenger van service, to and from the offices of physicians and of other providers of health care and medical services — such transportation being restricted to those passengers who will receive, or who have received medical treatment and services at such facilities – between points and placed in Calhoun, Doddridge, Gilmer, Harrison, Jackson, Marion, Monongalia, Pleasants, Preston, Ritchie, Roane, Taylor, Tyler, Wetzel, Wirt and Wood Counties, on the one hand, and points and places in West Virginia, on the other hand, subject to the following exclusion: the transportation of those individuals who have medical needs greater than those of the average population and who are expected to require the presence of a trained emergency medical technician during transport. The Commission does not have jurisdiction over the provision of emergency medical services, which lies with the Commission of the West Virginia Bureau of Public Health.
Rates:
$1.85 per loaded mile
$0.50 per unloaded mile, after the first 10 unloaded miles and an alternative rate âas established or agreed to by the West Virginia Department of Health acd Human Resources (WVDHHR) and /or its agents, contractors, brokers or other authorized representatives.ã
IT IS THEREFORE ORDERED that G & B Transportation, LLC give notice of the filing of the application, by publishing a copy of this order once in a qualified newspaper as provided in W. Va. Code 559-3-1 a,pub lished and of general circulation in Calhoun, Doddridge, Gihner, Jackson, Pleasants, Ritchie, Roane, Tyler, Wetzel, Wirt and Wood Counties, making due return to this Commission of proper certification of publication immediately after publication. Anyone desiring to protest and/or intervene should file a written protest and/or notice of intervention within ten days following the date of this publication unless otherwise modified by Commission order. If no protests are received, the Commission may waive formal hearing and render its decision on the application based on the evidence submitted with the application and the Commissionás review thereof. Requests to intervene must comply with the Commissionás rules on intervention set forth in the Commissionás Rules of Practice and Procedure.
