Box 69, 622 Pax Branch Road, Pax, WV 2590 has submitted an application to the Department of Environmental Protection (DEP), Division of Mining and Reclamation (DMR) 1159 Nick Rahall Greenway, Fayetteville, WV, 25840 for a significant incidental boundary revision to Article 3 permit number O-6011-86 in order to add 177.24 acres in order to bond existing haulroads on adjacent permits and delete 11.85 acres from the current permit area. The revised permit will discharge into Long Branch and unnamed tributaries of Long Branch of Fifteenmile Fork, Abbott Creek of/and Fifteenmile Fork of Cabin Creek, unnamed tributaries of/and Cabin Creek, unnamed tributaries of/and Skitter Creek of Paint Creek, and unnamed tributaries of/and Left Fork of White Oak Creek, unnamed tributaries of/and Fulton Creek, unnamed tributaries of/and Long Branch of Clear Fork all of Clear Fork of Big Coal River of Coal River of the Kanawha River Watershed and is located 5 (miles), northeast of Dorothy in the Cabin Creek District of Kanawha County, Valley District of Fayette County, and Clear Fork District of Raleigh County, Longitude 81° 25' 16" and Latitude 38° 01' 04" (Coordinates from USGS Topographic Map).

Written comments and/or requests for an informal conference of the surface mining application shall identify the applicant and application number and will be received by the Permit Supervisor at the DMR office and address above until March 21st, 2026, or thirty (30) days from date of final publication.
