No.: WVG611011 – MOD 2 Applicant: MPLX TERMINALS LLC 539 SOUTH MAIN ST FINDLAY, OH 45840 Location: KENOVA, WAYNE COUNTY Latitude: 38:24:32 Longitude: 82:35:09 Receiving Stream: Ohio River Activity: To add an additional outlet to the already permitted site. This outlet will be identified as outlet 6. Business conducted: Petroleum bulk terminal with pipeline transfers and marine loading. Implementation: N/A. On the basis of review of the application the “Water Pollution Control Act (Chapter 22, Article 11-8(a)),” and the “West Virginia Legislative Rules,” the State of West Virginia will act on the above application. Any interested person may submit written comments on the site registration permit application modification and may request a public hearing by addressing such to the Director of the Division of Water and Waste Management within 30 days of the date of the public notice. Comments received within this period will be considered prior to acting on the permit modification application. Correspondence should include the name, address and the telephone number of the writer and a concise statement of the nature of the issues raised. The Director shall hold a public hearing whenever a finding is made, on the basis of requests, that there is a significant degree of public interest on issues relevant to the draft site registration permit modification application and this facility’s coverage under the General Permit. Interested persons may contact the Public Information Office to obtain further information. The application, draft permit modification may be inspected, by appointment, at the Division of Water and Waste Management Public Information Office, at 601 57th Street SE, Charleston, WV, between 8:00 a.m. and 4:00 p.m. on business days.
