Pitrolo, Jr., Chairman of the West Virginia Health Care Authority, today announced that the Authority has received a request for an exemption from Certificate of Need review, pursuant to West Virginia C.S.R. c 65-27, from Summers County ARH Hospital, Hinton, Summers County, CON File #15-1-10565-X, for the acquisition of a rural health clinic in Summers County. The facility is located at 197 Pleasant Street, Hinton, Summers County. The office will be staffed by one family physician, one physician assistant and one nurse practitioner. The capital expenditure associated with the project is $50,000. TRAFFIC SIGNAL PROJECT
MONONGALIA COUNTY, WEST VIRGINIA

Separate, sealed bids will be received by the City of Morgantown, for the University Avenue and Oakland Street Traffic Signal Project at the Morgantown Municipal Building, 389 Spruce Street, Morgantown, WV, Monongalia County, West Virginia, 26505 until 2:00 pm prevailing time on Tuesday, August 11, 2015 at which time bids will be publically opened and read aloud.

The project consists of the installation of a traffic signal and ADA curb ramps at the intersection of University Avenue and Oakland Street. This includes new powder coated signal mast arms, black LED vehicular heads and countdown pedestrian signal heads, a traffic signal controller assembly, audible pedestrian push buttons, ADA curb ramps on all four (4) corners of the intersection, new pavement markings, and signage related to the new signal operation. Work at the adjacent intersection at University Avenue and Alumni Drive includes the installation of a GPS clock to provide a coordinated signal system along University Avenue.
The Bidder, if successful and awarded a Contract, agrees that they can begin construction on Friday, August 21, 2015 and will be Substantially Complete on Friday, December 18, 2015. The contractor further acknowledges that all work, including punch list items, removal of all construction equipment, restoration of temporary construction facilities, final cleanup and project closeout will be completed by Monday, January 18, 2016.

A mandatory pre-bid meeting will be held on Thursday, August 6, 2015 at 10:00 a.m., at the City of Morgantown Municipal Building, 389 Spruce Street, Morgantown, WV 26505. Bidders are required to attend the pre-bid meeting.

A two-envelope system will be used. Each bid must be submitted in a sealed envelope. Envelope No. 1 must contain the Bid Bond, Certification of Compliance, Power of Attorney, and all other necessary documents required on the Bid Opening Checklist. Envelope No. 2 must be a separate, sealed envelope marked "Bid Proposal" and must contain the completed and signed Bid Proposal. Both envelopes must have the following information in the lower left hand corner.

Name and address of Bidder, Bidder’s WV Contractor’s License Number, “Bid Call 2016-02 University Ave. and Oakland St. Traffic Signal Project” , City of Morgantown, 389 Spruce St., Morgantown, WV, Monongalia County, West Virginia 26505. To be opened on Tuesday, August 11, 2015.

The main envelope "Envelope No. 1" will be opened first and the Bid Bond, Power of Attorney, Certificates of Compliance and other pertinent and required documents as listed on the Bid Opening Checklist will be checked to ensure that all documents are in order. If the documents are found to be in order, the "Envelope No. 2" containing the Bid Proposal will then be opened and publicly read aloud. If the documents are not in order, the envelope containing the Bid Proposal will not be opened and the Bid will be considered non-responsive and will be returned to the Bidder unopened.

A bidder may not modify or withdraw his/her Bid or a period of five (5) days after the date set for the opening of bids.

Bids shall be accompanied by a Bid Bond or certified cashier’ s check payable to the Town of Star City in an amount equal to five (5{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}) of the base bid.

All bidders must hold a current West Virginia Contractor’ s License on the date of the Bid Opening.

Bids received after the scheduled closing time for the reception of bids will be returned unopened to the bidders.

Bidders must comply with the Title VI of the Civil Rights Act of 1964, the Copeland Anti-Kickback Act, the Contract Work Hours Safety Standards Act, and the WV Act on Wages for Public Improvements as revised by Senate Bill No. 361 effective on April 13, 2015.

The City of Morgantown, reserves the right to reject any and all bids and to waive any informalities.
