Application for a permit to operate as a contract carrier for
MTM, Inc., providing non-emergency medical transportation
in Brooke, Hancock, Marshall, Monongalia, Ohio, and Wetzel
Counties.
NOTICE OF APPLICATION FOR CONTRACT CARRIER AUTHORITY AND HEARING
On September 28, 2016, the Applicant, Three Rivers Transportation, LLC., filed an application for a permit to operate as a contract carrier for MTM, Inc., providing non-emergency medical transportation in Brooke, Hancock, Marshall, Monongalia, Ohio, and Wetzel Counties.
The proposed certificate will provide that the Applicant has permission:
To operate as a contract carrier by motor vehicle in
the transportation of passengers for non-emergency
medical transport in Brooke, Hancock, Marshall,
Monongalia, Ohio, and Wetzel Counties, West Virginia.
Rates:
SEDAN/SUV-ONE WAY TRIP RATES:
Base rate of $10.00 for all loaded miles from the point of pick-up
to the point of drop off. Mileage Rate applies in addition to the
base rate on all loaded miles after the first five miles from the
point of pick up to the point of drop off at a rate of $1.25
per mile. Mileage rate per one-way trip after 15 miles from
Appilcant’s main facility or substation to the pick-up point will be
calculated at $0.75 per mile.
WHEELCHAIR VAN-ONE WAY TRIP RATES:
Base rate of $20.00 for all loaded miles from the point of pick-up
to the point of drop off. Mileage Rate applies in addition to the
base rate on all loaded miles after the first five miles from the
point of pick up to the point of drop off at a rate of $1.25
per mile. Mileage rate per one-way trip after 15 miles from
Applicant’s main facility or substation to the pick-up point will be
calculated at $0.75 per mile.
On January 3, 2017, the matter was set for hearing on February 23, 2017, at 11:30 a.m. in Morgantown’s City Building, Council Chambers, 389 Spruce Street, Morgantown, West Virginia.
Anyone wishing to protest the above application or to intervene in the above styled matter should do so, in writing, to the Commission’s Executive Secretary of the West Virginia Public Service Commission, P.O. Box 812, Charleston, West Virginia 25323, within ten (10) days of the date of publication.
