PENDLETON COUNTY PUBLIC SERVICE DISTRICT

Application for a certificate of convenience and necessity to construct certain additions and improvements to its existing waterworks system.

NOTICE OF FILING

On May 25, 2016, Pendleton County Public Service District (District) filed a duly verified application for a certificate of convenience and necessity to construct certain additions and improvements to its existing waterworks system.

The District proposes to construct certain additions and improvements to its waterworks system consisting of: (i) installation of approximately 43,486 linear feet of 8-inch water line, 5,639 linear feet of 6-inch water line, 3,600 linear feet of 4-inch water line and 2,177 linear feet of 2-inch water line; (ii) installation of one 2-inch/6-inch PRV station; (iii) installation of one 105,000 gallon water storage tank; and (iv) installation of all necessary mechanical, electrical, piping, valves, controls and appurtenances related thereto. The project will allow the District to provide new water service to approximately 129 new customers in the Seneca Rocks area of Pendleton County.

The application is on file with and available for public inspection at the Public Service Commission, 201 Brooks Street, Charleston, West Virginia.

The estimated cost of the project is $2,850,000. The District proposes to fund the project with a $500,000 loan from the United States Department of Agriculture (USDA) for a period of 40 years at 3.5{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c} interest, a $850,000 USDA grant, and a $1,500,000 Small Cities Block Grant.

No rate increase is required for the project. The District will continue to charge its existing rates to its existing and new customers as follows:

SCHEDULE I

APPLICABILITY

Applicable within the entire territory served.

AVAILABILITY

Available for general domestic, commercial, industrial and resale water service.

RATES (customers with metered water supply)

$13.61 per 1,000 gallons

MINIMUM CHARGE

No bill will be rendered for less than the following based on meter size:

5/8 inch meter – $27.21 per month

3/4 inch meter – $40.82 per month

1 inch meter – $68.03 per month

1 1/2 inch meter – $136.05 per month

2 inch meter – $217.68 per month

3 inch meter – $435.36 per month

4 inch meter – $680.25 per month

6 inch meter – $1,360.50 per month

8 inch meter – $2,176.80 per month

DELAYED PAYMENT PENTALTY

The above schedule is net. On all accounts not paid in full when due, ten percent (10{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}) will be added to the net current amount unpaid. This delayed payment penalty is not interest and is to be collected only once for each bill where it is appropriate.

RETURNED CHECK CHARGE

A service charge equal to the actual bank fee assessed to the District up to a maximum of twenty-five dollars ($25.00) will be imposed upon any customer whose check for charges is returned by the bank due to insufficient funds.

RECONNECTION FEE – $20.00

To be charged whenever the supply of water is turned off for violations of rules, nonpayment of bills or fraudulent use of water.

TAP FEE

The following charges are to be made whenever the utility installs a new tap to serve an applicant:

A tap fee of $350.00 will be charged to all customers applying for service outside of a certificate proceeding before the Commission for each new tap to the system.

SECURITY DEPOSIT

Not to exceed two-twelfths (2/12) of the average annual usage of the applicant’s specific customer class, or fifty dollars ($50.00), whichever is greater. This fee may change by applicable statutory provisions.

LEAK ADJUSTMENT RATE

$1.98 per 1,000 gallons is to be used when a bill reflects unusual consumption which can be attributed to eligible leakage on the customer’s side of the meter. This rate

shall be applied to all such consumption above the customer’s historical average usage.

SCHEDULE II

Applicable within the entire territory served.

AVAILABILITY

Available for private fire protection service.

RATES

Where connections, hydrants, sprinklers, etc., on property maintained by

consumer:

2-inch line w/hydrants, sprinkler, and/or hose connections $6.10 per month

3-inch line w/hydrants, sprinkler, and/or hose connections $6.41per month

4-inch line w/hydrants, sprinkler, and/or hose connections $8.54 per month

6-inch line w/hydrants, sprinkler, and/or hose connections $28.07 per month

8-inch line w/hydrants, sprinkler, and/or hose connections $37.63 per month

10-inch line w/hydrants, sprinkler, and/or hose connections $42.71 per month

12-inch line w/hydrants, sprinkler, and/or hose connections $60.61 per month

These rates represent the existing rate schedules of the District. The District does not currently have any resale customers.

Anyone desiring to protest or intervene in the Certificate proceeding should file a written protest or request to intervene within thirty (30) days following the date of this publication unless otherwise modified by Commission order. Failure to timely protest or intervene can affect your right to protest aspects of this certificate case or participate in future proceedings. All protests or requests to intervene should briefly state the reason for the protest or request to intervene. Requests to intervene must comply with the rules on intervention set forth in the Commission Rules of Practice and Procedure. In the absence of substantial protest received within thirty (30) days of this publication, the Commission may waive formal hearing and grant the application based on Commission review of the evidence submitted with the application. 1c
